ULEM License Plate
We have started taking license plate applications again! Download the plate application here. Section 1 should already read “Utah Law Enforcement Memorial” and “Utah Law Enforcement Memorial Inc” in the two boxes. The rest of the boxes are for your personal and vehicle information.

Please make sure you fill it out as completely as possible. We will be relying on the contact info you put on this form if we find any issues with your application and need to contact you about it.

If you are wanting to personalize your license plate you can use up to 5 characters, not 7. You will need to send in the personalized plate form along with the plate application.

For a regular plate, send a check or money order for $54.00. For a personalized plate, send a check or money order or $99.00.

Attached is the basic mock up of the license plate and you can click it to view a larger version. The line at the bottom will most likely be changed to be more like a flowing ribbon and our logo will look a bit better than portrayed here.

    I’ll have mine in the mail Monday. How close is this to happening? I’m a retired Sgt. from Southern CA and would love to have this plate on my rig.

    Just purchased a new vehicle and won’t have plates for another month. Do I need to wait until I have plates to apply for this plate or can I do it now? Would love to have this on both my new vehicle and my jeep.

    Krystal, you will need to register your car like normal. Then send the application and check for $54 to the address listed. The plates wont’ be available until 3-5 months after we have our 500 applications turned in.

    MaryJo, at the current time we are still collecting the initial 500 applications we need. We are just over 1/2 way there with apps slowly trickling in. Delivery won’t be for 3-6 months AFTER we have received and turned over the initial 500 apps to the Utah Tax Commission.

    Danny, Are the plates still available. I would like to send my information in? I am just curious, do you have enough applications to be able to get the plates? If not I would be willing to share with friends or on social media? I am a law enforcement officers wife and a ex dispatcher so I have much respect for this cause and I really want these plates on my car. Please let me know.

    Thank you for reaching out to us. Unfortunately, both are considered “Special Group” plates vehicles are only allowed one “Special Group” plate. Thank you for thinking of us.

    Yes, there is an additional charge of $54.00 for a regular plate and $99.00 for a personalized plate.

    I’ll be sending an application in shortly. Thank you to ksl for doing a piece on this or I wouldn’t have known about it. In regards to the line at the bottom, you mention it will probably be changed. This is only my humble opinion, but I think it looks great the way it is!

    I have an SLCPD Detective son so will definitely want a plate. I hope they don’t change the “thin blue line” shown on the plate to the ribbon alluded to. The thin blue line is iconic and appropriate.

    You do not have to send in another application; however, you do have to send in the payment ($54 or $100 for a personalized plate).

    Is there any way you can send out a mass email to all or most Utah law enforcement agencies; and/or place it on the DMV website as a pending plate? I’m sure it would speed up this process.

    Most people are not aware this license plate is a possible option.

    I sent in an application almost 2 years ago and have heard nothing sense. A couple of phone calls to you and two texts recently have gone unresponded to. I guess you are a busy guy. My grandfather, a Utah Sheriff, was killed in the line of duty years ago and I have decided to get a Fallen Hero plate. Please remove my application from your records as I am no longer interested.

      I apologize for the delay with the plates. We are at about 250 applications and we need 500 applications in order to move forward. We do our best to be prompt with our responses. A member of our Executive Board will be in contact with you; so, we can figure out where the communication break down occurred. We are dedicated to honoring fallen officers, such as your grandfather, and we will do everything we can to make sure he is not forgotten.

    At the ride this year, you said they are now available for Motorcycles, the downloadable application says No Motorcycles.
    Is there going to be an updated application soon? Or do we just use this application?
